Deadline for submitting medical paperwork is August 1st
Athletic Training Pre-Participation Requirements:
All NCAA student-athletes at University of Scranton are required to provide the athletic training staff with pertinent medical information prior to their participation in a varsity athletics program. The athletic training staff uses the Athletic Training System (ATS) for online record keeping and injury documentation. Each athlete must first create a profile on ATS and upload the required forms into their ATS profile. Once all forms are uploaded, they will be reviewed by the athletic training staff, and the athlete will be cleared to participate once all paperwork has been verified. All ATS requirements must be submitted by August 1st. Failure to submit required paperwork by August 1st may result in a delay in the athlete being cleared for participation.
Required Forms Include:
· Pre-participation Physical (must be dated no more than 6 months prior to start of season)
· Policies and Procedures Acknowledgment
· Sickle Cell Trait Test Results
· HIPAA Medical Release Form (will be completed upon arrival to campus)
· Waiver of Liability
· Authorization of Release of Medical Information
· Medical History Questionnaire
ATS Login:
All new athletes need to create a user profile in ATS in order to upload necessary medical paperwork prior to sports participation. New athletes are required to enter demographic information, medial history, insurance, and emergency contact information. Incoming first years and transfer students-athletes will use “new” for both the username and password to gain initial access. Once logged in, athletes will need to create a new username and password and go through each tab and fill out all required information. The required forms are uploaded to ATS under the E-Files tab.

INSTRUCTIONS FOR ALL STUDENT-ATHLETES:
The "FORMS" tab is where all athletes will submit their medical history. The medical history contains 2-3 pages of Y/N questions. Each athlete will hit the "save" button once completed. This piece of the paperwork is not to be mistaken with the "medical history" tab where clicking the "verify" button is all that is needed. These are not one in the same.

Pre-Participation Physical Exam Guidelines:
Every University of Scranton student-athlete must have a pre-participation physical exam (PPE) on file with the Department of Sports Medicine that is signed by a physician, physician assistant, or nurse practitioner and indicates that the student-athlete can participate in intercollegiate athletics. The PPE must have been performed within six months prior to the submission of the form on ATS and are required for all new team members (first-years and transfer students). The PPE must be dated. Annual medical histories are required for subsequent years.
Sickle Cell Test Results
Effective August 1, 2022, the NCAA is mandating that its member institutions verify Sickle Cell Trait status of every student-athlete (including those who try-out). All student-athletes at The University of Scranton are required to provide documentation demonstrating the presence or absence of sickle cell trait. Typically, this test is performed on all newborns in the United States; however, those records may be difficult to access. The sickle cell trait waiver will no longer be accepted and student-athletes will be unable to participate in athletics at the University of Scranton until they submit their sickle cell trait test results.
